The Joint Commission has put out a Sentinel Event Alert on unsafe practices tied to injection therapies. The CDC has found 49 cases of hepatitis in patients undergoing injection therapies, and the Joint Commission has found many cases in its Sentinel Event Database of the spread of hepatitis and other blood-borne pathogens due to unsafe practices with single-use and multi-use vials for injection therapies. In a survey of healthcare providers, the Joint Commission found that 15% of providers used the same syringe multiple times on the same patient with multiple use vials, and of that number, 6.5% used multiple use vials on more than one patient.
